Development of review indicators and obstacle factors analysis of immunosuppressant medication compliance management in kidney transplant patients

Objective To comprehensively evaluate the current clinical application status of evidence regarding immunosuppressive medication compliance management in kidney transplant patients,construct review indicators,analyze the obstacles and promoting factors,and further formulate reform strategies. Methods Using the Joanna Briggs Institute evidence-based health care model as the theoretical framework,clinical nursing issues were identified,and a systematic search,evaluation,and summarization of 38 items of evidence were conducted. An evidence-based practice group was established;review indicators were constructed;review methods were clarified. Baseline reviews of systems,healthcare professionals,patients,and their families were conducted from November 1,2022,to January 31,2023. According to the review results,the obstacles and promoting factors in the process of evidence-based practice were analyzed,and corresponding strategies were formulated. Results A total of 23 review indicators were constructed. Among them,the accurate execution rate of 15 indicators was less than 60%,and the accurate execution rate of 4 indicators was 0. The main obstacles include a lack of systems and processes in departments,poor knowledge,attitude,and practice of medical staff,a lack of standardized risk assessment,and a lack of information support for out-of-hospital management. The primary promoting factors are strong organizational leadership,multidisciplinary team support,and high participation enthusiasm of recipients and their families. Corresponding reform strategies are formulated accordingly,including process and system improvement,continuous quality monitoring,enhanced standardized training and assessment,electronic health intervention for medication compliance,and involving patients and their families in the medication compliance management process. Conclusion There is a significant gap between the evidence on immunosuppressive medication compliance management in kidney transplant patients and its clinical practice. It is essential to assess the obstacles and facilitators scientifically and comprehensively in clinical situations,employ targeted reform strategies,facilitate the translation of evidence into clinical practice,and enhance the quality of nursing care.

The application of mindfulness-based intervention in nurses’ compassion fatigue:a scoping review

Objective To analyze and summarize the strategies to improve nurses’ compassion fatigue through reviewing the application research of mindfulness-based intervention,to provide the guidance for clinical medical staff to carry out mindfulness-based intervention and related research. Methods According to the research method of scoping review, a literature search was performed in the CNKI, Wanfang, CBM, PubMed, Embase, CINAHL, Web of Science,Scopus,ProQuest, the Cochrane Library. The retrieval time limit was from the establishment of the databases to June 15,2022. The included studies were summarized and analyzed. Results A total of 26 articles were included. The main elements of mindfulness-based interventions include curriculum theory introduction,mindfulness skills training,compassion training,stress coping guidance and mindfulness commitment practice. The main forms of mindfulness intervention are offline training,online training and mixed training. The frequency and length of training for the theoretical course of mindfulness intervention is mostly once a week,and 1.0~2.0 h per session;the frequency and length of skill training is mostly 2~4 times per week,and less than 30min per session. The outcome evaluation indicators include application effectiveness,feasibility,and application experience. Conclusion Mindfulness-based intervention has a variety of content elements and implementation strategies,and has positive application effects on nurses’ compassion fatigue. In the future,we need to continuously optimize the content and strategies of mindfulness intervention by combining the Internet and intelligent technologies,and conduct more high-quality randomized controlled studies to verify their long-term intervention effects.

Objective To comprehensively search the relevant research of Ottawa Decision Support Framework (ODSF) and to clarify the specific connotation of its constructs,understand the areas covered by its clinical application,and to analyze its specific role in the research.Methods Using the method of Arksey and O’Malley scoping review,we systematically searched CNKI,SINOMED,Wanfang,VIP,PubMed,Cochrane Library,CINAHL,Embase,Web of science,and screened the literature according to the inclusion and exclusion criteria. 2 researchers independently extracted the key information of the ODSF. Descriptive analysis and topic analysis were used to classify the extraction results.Results A total of 143 documents are included. There are 3 types of conceptual descriptions of ODSF in previous studies,which correspond to the 3 versions of ODSF. The framework covers a wide range of research areas,mainly including cancer,orthopedic diseases,cardiovascular diseases,etc. The guiding role of the framework in the research mainly has 7 categories,and there are obvious differences between domestic and foreign research.Conclusion This study summarizes the connotation and clinical application of ODSF in detail,which can provide guidance for future research.

Experience of the specialist nurse team in the multidisciplinary outpatient care on weight-loss

Objective To summarize the valuable experience of the specialist nurse team in the multidisciplinary outpatient care on weight-loss. Methods The team consists of diabetes nurses,nutrition nurses,rehabilitation nurses,psychiatric nurses and traditional Chinese medicine nurses,providing a comprehensive assessment,individualized weight management and follow ups based on metabolic,nutritional and behavioral aspects for patients in outpatient setting. Results Our specialist nurse team followed 47 patients for 6 months.Body weight,total cholesterol(TC),triglyceride(TG),low density lipoprotein cholesterol(LDL-C) and insulin resistance index(HOMA-IR) were significantly reduced compared to baseline. Moreover,the prevalence of unhealthy lifestyle,anxiety and depression was also significantly lower than baseline(P<0.05). Conclusion The specialized nurse team has provided the comprehensive and standard weight management for overweight or obese patients,which helps the patients to lose weight,improve anxiety,depression and metabolism state in the multidisciplinary outpatient care on weight loss.

Construction of a nursing-sensitive quality indicator system in wound and ostomy care

Objective To construct a localized,scientific,and reliable nursing-sensitive quality indicator system,in order to provide standardized tools for evaluating the quality of wound and ostomy care. Methods This study is based on the theory of "structure-process-result" quality evaluation model,using evidence-based analysis and semi-structured interview to formulate the consultation questionnaire,and the Delphi method and Analytic Hierarchy Process are employed to determine the content of the system and the weight of each indicator. Results The effective recovery rates of the 2 rounds of questionnaires were 94.1% and 100%;the authority coefficients of experts were 0.897 and 0.931;the Kendall harmony coefficients of expert opinions were 0.110~0.156,0.162~0.196 respectively. Finally,a nursing-sensitive quality indicator system of wound and ostomy care was constructed,including 3 first-level indicators,8 second-level indicators,and 20 third-level indicators. Conclusion The system shows strong scientificity,as well as excellent reliability and clinical applicability,which is consistent with the characteristic of clinical practice and nursing management of wound and ostomy care,and it can provide references for objectively evaluating the quality of specialist care.

Construction of a core competence evaluation index system for nurses participating in ECMO technology

Objective To construct a scientific and systematic core competence evaluation index system for nurses participating in ECMO technology,in order to provide references for the training,assessment and competence evaluation of nurses. Methods Through literature review and semi-structured interviews,the preliminary draft of the core competence evaluation index system for nurses participating in ECMO technology was formed in this study. From February to April 2021,25 experts from 6 provinces in China were consulted by Delphi method,and the weight of indexes at all levels was determined by analytic hierarchy process. Results The effective recovery rates of the 2 rounds of expert consultations were 90.00% and 92.59%,respectively;the expert authority coefficient was 0.886,and the Kendall harmony coefficient was 0.205(P<0.001) in the second round. The final core competence evaluation index system for nurses participating in ECMO technology includes 7 first-level indexes,19 second-level indexes and 70 third-level indexes. The first-level indexes include professional knowledge,clinical practice ability,critical thinking ability,professional development ability,communication and coordination ability,management ability,and personal traits. Conclusion The core competence evaluation index system for nurses participating in ECMO technology in this study was scientific and reliable,which can provide valuable references for the training,assessment and competence evaluation of nurses.

Design and clinical application of a special oxygen mouth guard for endoscopy

Objective To design a special oxygen mouth guard for endoscopy and to evaluate its clinical application effects. Methods The designed special oxygen mouth guard comprises 7 elements(such as mouth pad,nasal and oral oxygen inhalation tubes). A total of 120 patients with common bile duct stones undergoing endoscopic retrograde cholangiopancreatography at the endoscopy center of a tertiary hospital in Shanghai from January to February 2021 were included,and they were randomly divided into an experimental group and a control group,with 60 patients in each group. In the experimental group,the special oxygen mouth guard was used. In the control group,traditional oxygen tube and mouth guard were used. The wearing time,oxygen saturation(SpO2) fluctuations,nasal catheter displacement rate and medical staff satisfaction were evaluated and compared between 2 groups. Results The wearing time in the experimental group was(20.58±2.43) s,while it was(27.30±2.89) s in the control group,and the difference was statistically significant(t=-13.795,P<0.001). The SpO2 value in the experimental group was 2.00%(1.00%,2.75%),while it was 3.00%(2.00%,4.00%) in the control group,and the difference was statistically significant(Z=-4.332,P<0.001). The nasal catheter displacement rate in the experimental group was 0,while it was 18.33% in the control group,and the difference was statistically significant(χ2=-12.110,P=0.001). The score of medical staff satisfaction in the experimental group was(28.35±1.39),while it was(26.48±1.56) in the control group,and the difference was statistically significant(t=6.933,P<0.001). Conclusion The special oxygen mouth guard for endoscopy is easy to wear,and it can reduce the fluctuations in SpO2 and nasal catheter displacement rate,and improve the medical staff satisfaction and safety of ERCP.

Systematic review of pressure injuries risk prediction models in ICU patients

Objective To conduct a systematic review of pressure injuries risk prediction models in ICU patients,in order to provide evidence-based references for clinical nurses to conduct pressure injuries prediction models. Methods Cochrane Library, PubMed, Embase,CINAHL, Web of science,CNKI,VIP,Wanfang,and Chinese bio-medical databases were searched for the related studies on pressure injuries risk prediction models in ICU patients from the establishment of the databases to December 31, 2020. 2 researchers were selected to retrieve studies according to the inclusion and exclusion criteria, and critically evaluated the quality of studies based on the Checklist for Critical Appraisal and Data Extraction for Systematic Reviews of Prediction Modelling Studies. Results 15 studies were included, containing 28 pressure injuries risk prediction models. The AUC(area under ROC curve) of 20 models were all over 0.7. Gender,age,diabetes mellitus, mechanical ventilation,length of ICU stay were the most important predictors of pressure injuries. Conclusion Studies of the pressure injury risk prediction model in ICU patients had high research quality, and the risk prediction models also had good predictive performance. Male, the aged, history of diabetes,mechanical ventilation and long stay in ICU were high risk factors for pressure injuries in ICU patients. Clinical nurses should choose models with better predictive performance based on clinical practice, so as to provide targeted preventions for patients.

There is a large number of multi-structure data in the management of pressure injury. The integration has gradually changed from cutting-edge technology to the actual application stage,and promoted the transformation of pressure injury management from "system management" to "data management". This article summarizes the application research of machine learning algorithms in pressure injury in the field of artificial intelligence from 3 aspects,namely application foundation,measuring and analyzing wounds,risk prediction model,and aims to promote the information management of pressure injury for references.

Objective To explore the core competence of ECMO specialist nurses in China and to provide standards for training ECMO specialist nurses. Methods With qualitative research methods,semi-structured interviews were conducted among 10 nurses who managed ECMO independently,3 ICU physicians,2 perfusionists and 3 head nurses. Content analysis was used to summarize and extract the theme. Results 7 themes were extracted,including professional knowledge,clinical practice ability,critical thinking ability,professional development ability,communication and coordination ability,management ability,and personal traits. Conclusion The core competences of ECMO specialist nurses are comprehensive. It is necessary to pay attention to and strengthen these 7 core competences of ECMO specialist nurses,so as to meet the needs of ECMO specialized nursing development.

The paper discusses the appli-cation status of Clinical Care Classification System in clinical nursing practice abroad from 3 aspects of applicability,mapping and compatibility,analyzes the application value of the system in clinical nursing practice,and provides thinking for its popularization and application in clinical nursing in China.

Evidence summary for clinical application of needle-free infusion connector

Objective To comprehensively retrieve and analyze evidence related to clinical application of needle-free infusion connector,and summarize the best evidence. Methods The databases of evidenced based resources were searched by computer,including BMJ best practice,Uptodate,et al. In addition,complementary search of the comprehensive databases in both Chinese and English was conducted to collect literature including clinical decision support system,evidence summary,guideline,expert consensus from January 2014 to May 2019. 3 researchers evaluated the quality of the included literature independently. With the help of external experts and the standardized data extraction,eligible literature was selected for analysis. Results 13 pieces of evidence were included with 1 clinical decision support system,5 evidence summaries,5 guidelines,and 2 expert consensuses. A total of 36 recommendations were obtained and classified into 6 different aspects,namely policy recommendations,disinfection recommendations,flushing and sealing pipe,type of joint and its replacement,and cautions in use. Conclusion Health caregivers and other medical staff should regulate their behaviors based on the evidence of the needle-free infusion connector,develop policy specifications for the clinical application of needle-free infusion connector,refine its management and training content,optimize the clinical behavioral standards for the selection,replacement,disinfection,and sealing of needle-free infusion connector in practice to ensure patient safety.

Objective To retrieve,appraise and summarize the best evidence of the insertion and maintenance of peripheral arterial catheterization among adult ICU patients. Methods We searched literature from the Registered Nurses' Association of Ontario(RNAO),National Institute for Health and Care Excellence(NICE),Scottish Intercollegiate Guidelines Network(SIGN),Guidelines International Network(GIN) and Chinese Medive etc.,coupled with database search including PubMed,CINAHL,Embase,UpToDate,Joanna Briggs Institute(JBI),CNKI,WANFANG to collect relevant guidelines,evidence summaries,recommended practice,and expert consensuses. The timeframe of data search ran from the inception of the databases to September 2019. 3 researchers evaluated the quality of the literature independently. With the help of external experts,eligible literature was selected for analysis. Results Totally 13 articles were selected,including 1 clinical decision,3 guidelines,7 evidence summaries,and 2 expert consensuses. Finally,27 items of best evidence were synthesized into 10 categories including education & training,catheter selection,catheter insertion and auxiliary technology,catheter label,skin sterilization and aseptic technique,dressing selection and replacement,transducers,pressure monitoring system,flush solution and replacement time. Conclusion This study summarized the best evidence for insertion and maintenance of peripheral arterial catheterization,and added supplemental evidence to standardize the management of arterial catheter in clinical practice,which may reduce the incidence of catheter-related complications and ensure the security of patients.

The outbreak of coronavirus disease 2019 brought new challenge to the hospital management. Nursing staff is the cornerstone to fight against the epidemics. It is vital to allocate and manage the nursing staff effectively and reasonably for prevention and control of the infectious disease. This article describes the construction of the management system,the assessment and allocation of nurses,the training and examining and performance management in a tertiary hospital in western China. The purpose of our study is to discuss the ideas and methods of the nursing staff management in the face of sudden epidemics,and to provide references and experience to manage nursing human resource effectively.
